Student Forms
Students are responsible for initiating requests, in writing, for exceptions and substitutions to degree requirements and University policies and procedures. Petition or request forms are designed to standardize and simplify the process of making requests.

- Certificate Registration Forms:
- Change of Campus Requests: During the summers, students may take courses at any Penn State location without requesting a change of campus.
- Chemistry Course Evaluation Form
- Concurrent Majors Application
- Extension to Qualify for a Major Request: For students who wish to request an additional semester to qualify for a major.
- General College Petition: This form must be used for requests dealing with the general education and admission-to-major requirements. All requests that pertain to general education requirements must be submitted before the beginning of a student's graduation semester. A department may also use the form for requests dealing with major requirements.
- International Course Evaluation Forms: Course Evaluation Form for non-Penn State courses, that have not yet been evaluated by Penn State faculty, that students plan to take abroad and a Course Usage Form to determine if and how certain courses taken abroad (evaluated as X99) may be used to meet Penn State degree requirements.
- International Travel Grant Application: To request funding support for expenses associated with a study or work abroad experience.
- Nondegree Continuation Request: To request the ability to continue to enroll in Penn State courses in nondegree (regular or conditional) status. This form is only relevant to students who were notified by the Registrar that college support is needed to be able to continue to take Penn State courses.
- Petitions to the Faculty Senate: Instructions for students who wish to request an exception to University policies or procedures.
- Reinstatement to Degree Status Request: For students in nondegree-conditional status who wish to request reinstatement to degree status in ENGR or a major.

- Access Account Extension for Student Leave of Absence: To request that the Penn State Access Account be maintained during an approved leave of absence.
