for exceptions and substitutions to degree requirements and University
policies and procedures. Petition or request forms are designed
to standardize and simplify the process of making requests.

Integrity: Expectations, policies, and procedures, and forms related to academic integrity in

- Certificate Registration Forms:
- Change of Campus
Request (from University Park): For students who wish to change from University Park to another
campus for a fall and/or spring semester. During the summers, students may take courses at any Penn State
location without requesting a change of campus.
- Concurrent
Majors Application
- Extension
to Qualify for a Major Request: For students who wish to request an additional semester to qualify for
a major.
- General College
Petition: This form must be used for requests dealing with the general education and
admission-to-major requirements. All requests that pertain to general education requirements must be
submitted before the beginning of a student's graduation semester. A department may also use the
form for requests dealing with major requirements.
- International Travel Grant
Application: To request funding support for expenses associated with a study or work abroad
experience.
- Petitions to
the Faculty Senate: Instructions for students who wish to request an exception to
University policies or procedures.
- Reinstatement to Degree
Status Request: For students in nondegree-conditional status who wish to request reinstatement
to degree status in ENGR or a major.
