Computer Access Form
- This form is to be completed only by all NEW students.
This form is necessary to get a Penn State computer access ID and
password. An ID and password is required to gain access to Penn State
computer resources including the Penn State Course Management System -
ANGEL. Please mail or fax the completed Computer Access Form to the address below. |
If you choose to register
for this course you will need to complete the registration form on-line
or call 1-866-570-7643. You will receive a tuition bill from the
University Registrar which may include an Information Technology Fee.
The Computer Access Form (if applicable) must be returned to Deb
Zimmerman.
