Computer Access Form - This form is to be completed only by all NEW students. This form is necessary to get a Penn State computer access ID and password. An ID and password is required to gain access to Penn State computer resources including the Penn State Course Management System - ANGEL. Please mail the completed Computer Access Form with your completed Registration Form to the address below.

If you choose to register for this course you will need to fill out the registration form and return it to the Program Coordinator, Deb Zimmerman along with payment of $2325.00. There is also an Information Technology Fee of $127.00. Check with Deb Zimmerman if you have any questions regarding this fee. You may pay by check, credit card, or bursar account.

Make checks payable to Penn State University.

The registration form must be returned to Deb Zimmerman.

Registration forms will be processed as a group for this course.
