Assignments

Registration and Evaluation Process

As a participant in the Co-op & Internship Program, you will be expected to complete a series of assignments that will be evaluated. At the beginning of each semester in which you are working, our staff will send you an e-mail message with information about how and when to complete the online Work Period Registration Form. Later in the semester, you will receive notification about the need for you and your supervisor to complete the online Midpoint Evaluation Forms; toward the end of the semester, you will be advised to complete the online Final Evaluation Forms. Due dates for each of the forms will be provided in the e-mail messages you will receive.

Below, please find the links to our online Work Period Registration Form and the Midpoint and Final Evaluation Forms. For each form, your userID and password will be sent to you when you are placed this semester. UserIDs and passwords for the supervisor evaluation forms will be sent to the students, and students will need to forward the e-mail to the person who is going to fill out the supervisor evaluation form.

Important for students: If you did not receive an e-mail with your userID and password for any of the forms, please contact our office. Please note that your password can be different for each form.

Online Registration and Evaluation Forms

To access each of our forms you will need a userID and password. You are registered as either a co-op or an internship student. When you logon to one of our evaluation surveys, please make sure that you have been directed to the correct form and contact our office in case you have any questions.

You can then choose the form you need to fill out. UserID and password to each of these forms will be e-mailed to you about two weeks before the forms are due.
