Narrative Statement (Page Limit: 1)

This statement must include a one paragraph description of the candidate’s job duties and history in the current position, work unit, and University before the candidate highlights his/her accomplishments.  Candidate must write in the third person and use standard margins and 12-point font.
 

Performance Evaluations

Candidate must include his/her two (2) most recent Staff Review and Development Plans (completed in 2007 and 2008).
 

PREP Criteria:  (written in the third person in either paragraph or bullet format)

Motivation and Initiative (Page Limit: 1)

Provide examples of candidate’s willingness to take on new projects proposed by his/her supervisor, department head, College, or University.  Describe any new projects or practices initiated by candidate.
 

Creativity and Innovations (Page Limit: 1)

Describe creative solutions to short- and long-term problems, innovations through the use of technology, or other efficient practices with a positive impact on the department/unit.  Examples of creativity and innovations can be formal (CQI Team) or informal.
 
 
 

Quantity (Page Limit: 1)

Provide examples of quantity of work performed.  Include specific examples of how efficient practices and/or experience result in a high volume of work completed (for a sustained period of time) and the impact of such practices on the department/unit (cost savings, staff streamlining).
 

Professional Development (Page Limit: 2)

Provide evidence of continuing commitment to improving the skills and knowledge required for existing duties and acquiring new information and skills that prepare the employee for future challenges.  Each activity must include a title and date(s), a brief description of the activity, and the candidate’s role in the activity (participant, presenter, etc.).  Please list in reverse order with most recent activities first.

Department/College/University Contributions and Services (Page Limit: 2)

Aside from activities required by the candidate’s job, list any service activities that aid the Department’s, College’s, and University’s goals.  Include dates and candidate’s role.

Immediate Supervisor’s Evaluation Statement (Page Limit: 1*)

This statement should focus on the PREP criteria plus any additional relevant information.
 

Department Head’s/Unit Leader’s Evaluation Statement (Page Limit: 1*)

This statement should focus on the candidate’s contributions to and impact on departmental/unit operations.  The evaluator may draw on both PREP and SRDP criteria.
 

* If the department head/unit leader is also the candidate’s immediate supervisor, the statements may be combined.  (Page Limit: 2)
 
 

PREP Committee’s Evaluation Statement and Recommendation
