If a faculty member is proposed for both promotion and tenure, one dossier should be prepared with two cover (signatory) pages, one cover page to document decisions on the tenure consideration and the other to document decisions on the promotion considerations. (Signatory page for tenure first, followed by the signatory page for promotion) In such cases the dossier which serves for both promotion and tenure should include all evaluative statements from previous probationary tenure reviews. (Administrative Guidelines, Section III.C.11) The Department Committee and Department Head must each prepare one evaluative statement which serves for both tenure and promotion. All statements must be in the combined promotion and tenure dossier. (Administrative Guidelines, Section III.C.11)

This item is completed only on promotion recommendations; it is left blank on tenure recommendations. Indicate the new rank desired for the Candidate.
Example: Associate Professor of Chemical Engineering.The Chair of the Department's Promotion and Tenure Review Committee must check the Committee's recommendation on the cover sheet, sign on the line entitled, "Chairperson-Dept. Review Comm.", and date the signature. The date of the chair's signature must be before the date of the Department Head's signature.
The Department Head must check his/her recommendation on the cover sheet, sign on the line "Department Head," and date the signature.
