Forms

       

Students are responsible for initiating requests, in writing, for exceptions and substitutions to degree requirements and University policies and procedures.  Petition or request forms are designed to standardize and simplify the process of making requests.  The forms and information listed below are available only in PDF format.  By clicking on the title, the desired form or information can be opened and printed.  Adobe Acrobat Reader must be used to read the PDF format and is available and free from www.adobe.com.  More comprehensive descriptions of these petitions, justifications for their use and other possible actions can be found at: www.engr.psu.edu/undergrad/adv/studentactions.html 

General College Petition Form: This form must be used for requests dealing with the general education and admission-to-major requirements.  All requests that pertain to general education requirements must be submitted before the beginning of a student’s graduation semester.  A department may also use the form for requests dealing with major requirements. 

Request for a Deferred Grade: If a student cannot complete course requirements because of circumstances beyond their control that happened at the end of the semester, he or she may request a deferred grade in the course(s).  Examples of such circumstances are: illness or accident during the final exam period, a family emergency at the end of the semester, etc. Official University forms are available in departmental offices and in the Engineering Advising Center in 208 Hammond Bldg., University Park. 

Form to Request a Change of Location (Assignment) from University Park: For students who wish to change from University Park to a campus college location for a fall and/or spring semester.  During the summers, students may take courses at any Penn State location without requesting a change of location.

Request Form for an Extension to Qualify for a College of Engineering Major: Students who are eligible to make such a request will be notified.

Instructions for Submitting Petitions to the Faculty Senate: For students who wish to request an exception to University policies or procedures.

Access Account Extension for Student Leave of Absence Form: To request that the Penn State Access Account be maintained during an approved leave of absence.
